Remove the door. Sliding doors can be lifted up and then out away from the track. For a single door attached separately to its frame, remove all the screws along the hinged edge, then remove the door.

Remove the top bar of the track by unscrewing any screws located at the ends and then lifting straight up.

Loosen and remove any caulk on the two side rails by gently manipulating a plastic putty knife along the edge. Remove all visible caulk. Remove any screws.

Remove the bottom rail in the same manner.

Clean any remaining residue left behind. Use the putty knife to aid you.

Install the new door according to manufacturer's instructions, or seal up existing holes with caulk.

Tips & Warnings
Try using a heat gun to soften stubborn caulk for easy removal. Most hardware stores carry colored caulk for an easy match.
Take extra precaution using the heat gun near a water source. Do not stand on the edge of a bathtub.
