You need to be licensed by the state to become a physical therapist in Indiana. But first you must fulfill academic requirements by getting a bachelor's degree in physical therapy from an accredited school. You can also enter rehabilitation therapy programs offered by graduate schools. Following courses, an aspiring physical therapist may also be required to do field work.

      Apply to take a state exam to receive your license. Complete and mail the application to Indiana's Professional Licensing Agency. The applications must include a $100 application fee made payable to the agency.       Physical therapy graduates must submit an examination fee of $350 to the Federation of State Boards of Physical Therapy (FSBPT). You will need to register on the FSBPT website, fsbpt.org.

      Take your test at Sylvan Learning Center, which operates the computerized testing on behalf of the state. You must also submit $65 to the testing center when you schedule your examination.

      Submit two passport-type photographs, an official transcript of grades from the school you attended and a notarized copy of a certificate or affidavit for a legal name change if the name differs from other documents.

      When you pass the exam, you will receive your license. Then you can begin to look for employment, if you have not already scheduled a job, at a center or clinic in Indiana.

Tips & Warnings

  • Temporary permits are available by paying a $50 application fee and a $100 fee for the temporary permit. A person with this permit may only practice under the supervision of a licensed physical therapist. The permit is good for 90 days or until the applicant becomes a certified physical therapist.

  • You should allow 2 to 4 weeks for the processing of the application for the exam. Make sure you have sent in all the necessary documents or it will take 2 to 4 weeks for you to be notified of the missing information by mail. Do not to phone the agency regarding approval or denial of the application. Graduates of foreign physical therapy programs must have a credentials evaluation from the state.
