      Write a paragraph on the action you want to be taken. Be clear and precise.

      Sign the petition and include your address and contact information. Be clear that you are responsible for writing the petition--and any questions should be directed to you.

      Leave a space to fill in the complete number of signatories. Include a statement that identifies this number.
