You can only get Death from the grave medals by killing an enemy player after you have already died. The best way to do this is to throw a grenade right before you die. Try throwing grenades at the ground or bounce them off of walls to kill your enemy after you have died. There are many different ways to get a Death From The Grave in Halo 3.

Use explosive items on the map to kill your enemies after you have died. Try throwing grenades at explosive barrels, forklifts, etc, right before you die to take out your enemies. There are many different items that you can use in Halo 3 to kill your enemies from the grave.

Start a matchmaking game on any playlist that allows the Mythic Maps. Find a game on orbital and keep trying these methods until you have 2 death from the grave medals. You will then unlock the Post Mortem Achievement on Halo 3 for the Xbox 360.
