Step 1
You can easily purchase a certified copy of your birth certificate right online. Be sure to have your mother's maiden name, your father's name, and the city you were born in for reference in filling out the information. You will also be asked your maiden name since this is what name your birth certificate will be listed under.

Go to the VitalChek website (see the resource list below) and follow the instructions listed to the right of the homepage under "start your order". You'll be asked for the information above and your date of birth, plus they will ask you for your reason for requesting the birth certificate. Click continue to go to the next page of your birth certificate order.

Step 3
You will get details on your birth certificate order, including the cost, which is usually only $18.00 unless you are purchasing more than one birth certificate or another certificate. Click continue. The next page will ask you for more detailed information including your maiden name and your parent's names (mother's maiden name). You must also know your parents' middle names.

Step 4
You will then need to verify the information you put in and click continue to the payment processing page. Enter your shipping address and credit card information and you are done! Hint: if you choose regular mail, your shipping for your birth certificate will be free, but may take up to 2 weeks. You can have it delivered within one to two days if you pay an extra $17.00.

Step 5
Be sure to print a copy of the order for your records to keep. An email will be send to your email address at time of order and when your birth certificate has been shipped. If you do not have a printer, you can save a copy of the email for your records.
