How to Slide a Note on Guitar
Sliding between notes on the guitar is a great way to add smoothness to any lick or phrase. It can also add new dimensions of expression and emotion to musical phrases and melodies. When executing a slide it is important to use proper technique to ensure that both notes have even tone and sustain.
Instructions

Place the left-hand index finger on the 5th fret of the 1st string.

Pluck the 1st string with a pick or right-hand fingers.

Slide the left-hand index finger up to the 6th fret. Do not lift the index finger off of the string; the sound should be continuous between both notes. Do not re-pick the second note.

Place the index finger back on the 5th fret of the 1st string.

Pluck the 1st string with a pick or right-hand finger.

Slide the index finger down to the 4th fret. Do not lift the finger and do not re-pluck the 1st string.

Practice playing slides on different strings, frets and with different left-hand fingers.

Tips & Warnings
Keep an even amount of pressure on the string when sliding to avoid any unwanted string noise or squeaks.
