1. Contact your state Department of Motor Vehicles (see Resources) to find out which forms you need to fill out. Check their website for links to the forms. Download the form and fill it out following the directions.

2. Call your local Department of Motor Vehicles if the forms are not available online and request them. Be sure to ask for detailed instructions and the items to include with the application.

3. Complete and sign the form and attach any necessary documentation. Some states require the form to be notarized. Be sure to include all pertinent information and provide proper identification. Include the replacement fee for the title.

4. Double-check the form for accuracy. Mail or take the form to the appropriate office.
