Step 1
Immediately after becoming disabled, you may apply for social security disability benefits by visiting the nearest social security office in your area, apply online or call their toll free telephone number.

Have your social security card available, verification of age and your current W-2 form on hand from your last job. You will need to apply also for an Adult Disability Report form. This form pertains to your employment profile and disability information.

Provide all medical documents such as locations of hospitals, telephone numbers of physicians and descriptions of medications that have been prescribed to you.

You may be asked to show your marriage certificate, if applicable. If your spouse is also applying for social security disability benefits, they will need to follow the same requirements.

After receiving approval to receive social security disability benefits, contact the social security office if you are able to return to work again. Your disability case will continuously be reviewed and you will be notified of any situations that may occur.
