How to Replace a Lost or Stolen Social Security Card
If you've lost or misplaced your Social Security card and need to replace it, it's very simple to do.

Choose which option you'd prefer to use. Option one is to use your home computer and printer to print out the necessary forms. You'll need form SS-5, which can be found online (link in resource links below). Option two is to do everything at your local Social Security office.

Use the proper kind of paper. If you choose to print out your Social Security card request form with your printer, it must be printed on 8.5" x 11" white paper. For those in other countries, where 8.5x11 paper is not available, A4 paper (8.25 x 11.7) is the only possible alternative to use.

Fill out the application carefully and legibly. The application must be written in print letters, and must be done with blue or black ink.

Take the printed form to your local Social Security office, along with your U.S. driver's license, a state-issued nondriver ID card, or your United States passport.

Here is a unique social security card tidbit of information. You can only have 10 cards issued in your lifetime. I found this out when I was recently married.
