Cut fabric to the measurements you have taken. Make sure to leave about an inch of extra fabric for a seam allowance.
Place the fabric with right sides next to the pillow filling and stitch with long basting stitches around the edges. Pulling the fabric tight so it will fit nicely when turned to the outside. YOu can pin this seam allowance first if you want to make sure it doesn't slip or maybe the fabric has stripes and you want to make sure they stay lined up. Leave an opening large enough to take the cover off after basting to size.

Next sew on the machine or by hand the basted edges of the cushion. If sewing by hand use a strong threat like button thread or dental floss. Since the stitches will not be as close as machine stitching. Clip the extra fabric off the edges and corners, and turn the inside out. Leave enough room to turn easily on the last seam edge. Turn the seam edges inside and whipstitch shut the opening.
Tips & Warnings
Remember that all the seams will turn to the inside when final stitching is done. so if it looks a little messy no one will be the wiser.
Try to hide the final stitches while whipping the edge closed. Make this on the side that will be on the back of the cushion.
Pick fabric that will hold up well with lots of use.
Fabrics with nap will smooth off, or wear down in areas that will make slick or uneven places after a lot of use.

I never would have thought to use dental floss, what a great idea!
