Step 1
Make it through the intro of you being born and choosing your character's specifications. You will have to choose how your character looks of course, so take your time with this -- you'll have to play with that mug for a long time if you are doing all of the quests in Fallout 3.

Step 2
When you become a teenager, your father will tell you that you have to take the G.O.A.T. test to decide what your responsibilities will be inside Vault 101. You begin in his office, and there on his desk will be the MEDICINE bobblehead.

Step 3
If you missed it the first time and are well into the game already and can't just start over, you get another chance to pick it up in the 'Trouble on the Homefront' side quest that you can get later in the game. Keep in mind, if you miss the MEDICINE bobblehead this time, you are out of luck!
