How to apply online to get services for a child with Down Syndrome or other Disability in Arizona

Finding Services such as Occupational, Speech, and physical therapy can sometimes be a difficult but extremely important aspect of raising a child with a disability. With all of modern technology it is now possible to start this using the internet.

Instructions

Things You'll Need:

  • Perseverance
  • Working internet connection or access to one.
  • A Child with a disability
  1. Step 1

    Ensure your child meets the guidelines for qualification for Department of Developmental Disabilities of Arizona.
    1.)You need to be a resident of the state of Arizona.
    2.)You have to voluntarily apply.
    3.)Is under six and at risk of having a developmental disability, or over six with a diagnosis of a disability.
    4.)Has at least three areas of delays that affect their daily living abilities.
    4.)The disability occurred before they turned 18.

    If you can say yes to all this then proceed to the next step.

    Obtain proof of the delay/disability from your child's pediatrician or family practitioner/doctor. If possible obtain proof from any specialists in child's disability that they are currently seeing.

    Proceed to the Arizona Department of Economic Security website at http://azdes.gov then click the link in the center of the page that says "Help individuals with Disabilities" with the wheelchair icon next to it.

    Next proceed to click on the Eligibility/Make a referral.

    Review the Eligibility requirements and decide if you would like to put in your application. If you decide not to put in an application you may still look for other resources to help your child by clicking the link provided on the page and specified in the image. Click continue to begin putting in your application.

  6. Step 6

    Complete the form as to the instructions and the helpful hints provided in the image.

  7. Step 7

    Click submit when finished and wait for the appropriate phone call to begin setting up services and an introduction meeting.

  8. Step 8

    Once the meeting is set up the Department of Developmental disabilities will evaluate your child accordingly and decide whether they require services or not. If they decide they do require services they will set up the appropriate goals with you and schedule evaluations for the appropriate services.

  • It never hurts to be too prepared for a meeting with any provider.
