How to File IRS Form W-9
Businesses use IRS Form W-9 "Request for Taxpayer Identification Number and Certification" to collect personal information from employees or customers for payment purposes. Corporations that do business with independent contractors and financial institutions that issue regular payments may ask individuals to complete Form W-9. On this form, taxpayers provide contact information as well as their employment status and an identification number.

Enter your name and business name, if applicable. When filling out Form W-9, use your name as it appears on your federal income tax return. If you conduct business under a different name or a company name, enter that on line 2.

Select the correct classification for your business. Designate the type of business you operate by checking the appropriate box. Individuals and self-employed businesspersons should typically select "Individual/Sole Proprietor," while owners of larger companies should choose the corporation class that applies to them. Complete the address section. List your full mailing address, along with your city, state and ZIP code.

Fill in the appropriate identification number. If you are an individual or are self-employed, enter your Social Security number as your taxpayer identification number (TIN) in Part 1. If you operate a larger business and you have received an employer identification number (EIN) from the Internal Revenue Service, enter this number as your TIN.

Sign and date the form. In Part 2, you must certify that the information you have provided is accurate, that you are not subject to backup withholding from the IRS, and that you are a United States citizen. If you meet all three of these conditions, sign your name and note the date.

Return the completed form to the business or institution that requested it. You should not submit Form W-9 to the IRS.
