The original use for the Social Security card and number was to track the funds in your Social Security account. When you get married and change your name, you must also let the Social Security Administration (SSA) know that you are using a different name. You do this by changing your name on your Social Security card. Not changing your name with the SSA may impact your tax refund and it may also impact your social security account and benefits.

From Quick Guide: Change Your Name
Difficulty: Easy
Instructions
  1. Step 1

Complete the form SS-5. This form can be downloaded from the SSA website. See the Resources section for a link. You can also call (800) 772-1213 to find out more information about where you can obtain the form.

  2. Step 2

Understand that you will need to prove that you are same person, even though your name may be different. Your marriage certificate counts as a legal document that shows your old name and your new name.

  3. Step 3

Know that you will need to prove your citizenship or immigration status. A U.S. drivers license, a state-issued identity card or a U.S. passport are acceptable proofs of documentation. If you are unable to obtain the suggested documents, SSA may accept the following documents: a school ID card, employee ID card, U.S. military ID card and a health insurance card. Keep in mind that a Medicare card is not acceptable.

  4. Step 4

Keep in mind that if you are a legal immigrant, you need to prove that you are allowed to work in the United States. The SSA will accept the following documents: Form I-551 (Green Card), I-94 (Arrival and Departure Record) and I-766 or I-688B (work permits) with an unexpired passport.

  5. Step 5

Understand that you will also need to prove your identity. The same documents that prove your citizenship immigration status can also be used to verify your identity.

  6. Step 6

Take the completed form and the original documents to your local Social Security office or mail the paperwork in. The SSA requires original documents or certified copies. SSA will not accept photocopies.

Tips & Warnings
  • Be sure to give your employer your new name to match all your pay and tax records.
  • Never carry your Social Security card in your wallet. Always keep it in a safe place.
