You have a few options when it comes to how you would like to begin creating your shirt. Photo Sourcing is a method where you can use a photograph of a piece of clothing, or a texture, to create your shirt. Another option you have is drawing your shirt by hand using the paint tools available with the program. When using a Photo Sourcing method, be sure that the image you use is not copyright, or that you have permission to use that image. Be wary of "free" image sites since some sources do have stolen textures.
