How to File a Pothole Damage Claim in New York City
They are the sometimes unavoidable craters of Gotham's city streets --gaping potholes that claim as their victims unsuspecting motorists and their vehicles. If you fall into one, do your fellow New Yorkers a favor and call 311 to report the pothole and then consider your options. You may be eligible for repair reimbursement from the city.

Note the location and date of the incident, including the exact location of the pothole.

Snap a picture of the damage to your vehicle and another of the offending pothole.

Obtain notarized statements from any witnesses to the incident, if possible.

Make a copy of the police report and any tow records or auto club reports, if applicable.

Make a copy of your license and registration to include with the report.

Go to the website of the New York City Comptroller, listed in the Resources section below.

Download an automobile property damage claim form from the site.

Fill out the form in its entirety. Be sure to include your social security number.

Include photos, reports, registration, license copy and copies of receipts and canceled checks for repairs or damage cost estimates.

Have the completed form notarized.

Submit the form in person or send it via certified mail to the New York City Comptroller's Office at One Centre Street, Room 1255, New York City, NY 10007.
