Instructions

    • 1

Ask your doctor if your disability qualifies you for a handicapped placard. He will know the criteria, which can include being unable to walk 200 feet, lung disease, portable oxygen, cardiac/neurological/orthopedic conditions or blindness.

    • 2

Obtain the application from the Ohio Department of Public Safety Bureau of Motor Vehicles or your doctor's office, or download it from the Internet. (See Resources.)

    • 3

Indicate on the application the type of placard you need, and note the fee of $3.50 you will need to submit with the paperwork.

    • 4

Get a prescription from your doctor, chiropractor or advanced-practice nurse. Be sure they submit an original prescription, not a copy, and follow the application guidelines precisely.

    • 5

Submit the application, prescription and fee to any local Deputy Register, or mail to the Ohio Bureau of motor Vehicles, Attn. Registration Section, PO Box 16521, Columbus OH 43216-6521.

    • 6

Hang the placard on the rear-view mirror when you park your vehicle. Remove it when driving.

Tips & Warnings

  • Be aware of any fines or penalties for misuse, and pay attention to the date your placard expires. You can also apply for a placard for an organization; the process and cost are approximately the same.

  • A handicapped parking placard is a necessity for people with serious disabilities. Do not abuse the privilege.
