Go to your local Social Security office and bring identification showing that you are a US citizen and prove your identity. The documents must be originals or certified copies. These can include a driver's license; state issued ID card, passport, military ID and school ID cards.

Mailing your card application is another option open to individuals who do not live in an area where their local Social Security Office is easily accessible. You will still need to include certified copies of your identification when you mail in a request.

Be aware that under most circumstances, you are only allowed to have your card replaced 3 times a year. For a lost or stolen card there is no charge for a replacement.
