How to Teach a Puppy to Go in the Same Spot Every Time
One way to help your puppy learn to be housetrained is to help him identify a spot where he can go. The best way to do this is with positive reinforcement on several levels. With a little patience, you will be able to teach your puppy to urinate in the same spot—in or out of the house, depending on whether or not you are using puppy pads—every time.

1
Wipe your puppy with a handkerchief the next time she urinates. This can be a little strange for you and the dog, but it will be well worth it, because it will provide a clear indication to your dog of where and what to do.
-
2
Select a "potty area." This should be close to the door if it is outside, in case you or your puppy are in a hurry. If you are using puppy pads for indoor training, just pick an appropriately convenient spot. Designate this area by placing the handkerchief on top of it.

3
Show your dog the handkerchief. Your dog will sniff it, remember what happened on it and almost always promptly re-anoint it. As soon as he does so, praise him effusively and give him a treat.
-
4
Replace the scent cloth in the same spot every time you take your dog outside. She will almost always opt to "go" right on or near the cloth. If she does not, do not scold her, but do not give her any positive reinforcement either. Eventually she will associate the cloth with both using the bathroom and rewards, and will not want to go anywhere other than in the designated spot.
-
5
Make sure you give your puppy every opportunity to access his potty area. The best way to set a precedent fast is to give your puppy lots of chances to do the right thing. Small dogs may need to go outside as often as every hour, while more mature puppies still need a chance at least every two to two and a half hours.

Tips & Warnings
There are many kinds of sprays that you can use to encourage or discourage a dog from going in different places. You may wish to use these in conjunction with your other housetraining techniques.
