Call (608) 261-2566 to begin checking your personal Wisconsin Driving Record. This is an automated service provided by the Wisconsin Department of Transportation. You will need either your license number or Social Security number to use this service, which costs $5 once you've completed the process. Be sure to listen to the entire message as you will be given instructions for obtaining your record based upon your individual needs (such as which address you want this information mailed to).

Log onto the “Request your own driving record” portion of the Wisconsin Department of Transportation website if you wish to obtain this information via mail.

Download the appropriate request form by clicking on the “MV2896” link found on the web page.

Fill out the form and mail it along with a check made out to the Wisconsin DOT to: Wisconsin Department of Transportation, P.O. Box 7995, Madison, WI 53707-7995.

Log onto the DMV website, the Unofficial Guide to the DMV. You should see a map of the United State on the home page.

Click on the state of Wisconsin to begin the process. Search for “WI” if you're unsure on the location of the state on the map.

Select “Driving Records” underneath the “What Can We Help You With” today heading found on the Wisconsin DMV portion of the DMV website. This will be the fifth and final link on the left side of the box found on that particular web page.

Choose the option which bests suits your needs. If you wish to obtain your own Wisconsin driving record, click the “Order Now” tab in the “For Individuals” box. This will redirect you to a third-party website which will require you to enter your license number, name (as it appears on the license), address and phone number. You will also need to enter billing information. Select the “Order Now” tab on the Wisconsin DMV portion of DMV.org in the “For Businesses” box if you wish to find a driving record for employment purposes. This will redirect you to a third-party website for business owners. You will need to sign up on this website to continue using the service. Although you will have to enter credit card information on this website, you will not be charged until you actually submit a request for a driving record.
