• Request for driver record information form

1. Go to your local Department of Motor Vehicles (DMV) office. Many offices encourage motorists to make an appointment for faster service.

2. Request your driving record. Complete the required application and provide identification. For example, motorists in California must complete a Request for Driver Record Information form (INF 1125).

3. Pay the fee. The fee for a driving record will vary, but most DMV locations accept cash, check, money order and ATM/Debit cards.

4. Check your driving record by mail. Mail the required request form to the DMV. In most cases, you must mail a Request for Driver Record Information form to the Department of Motor Vehicles/Office of Information Services. Obtain the correct address by contacting the DMV by phone. Include the necessary fee with the written request form.

5. Request your driver record with or without personal information. According to the Missouri Department of Revenue, "A driving record with non-personal information includes the driving history showing speeding tickets and other violations." A driving record with personal information might include the license holder's date of birth, Social Security number, photograph, telephone number and medical or disability information, among other things. DMV requirements will vary, but generally motorists must include their name, date of birth and driver's license number along with the type of record requested with the correct fee.
