Chief of Staff is the top notch in the Medical career ladder. Getting there requires lots of hard work, long hours and some very sharp skills. Though your Sim will spend a long time working their way up the Medical ladder, they will find life at the top quite enjoyable. Considering hours, vacation time and salary, the Medical career ranks among one of the best.

Become an Emergency Medical Technician. This is the first level of the Medical career track. It can be obtained through the newspaper, online job site or through cheating. Everybody has to start somewhere and this is where future Chiefs of Staff start. The pay isn't great, but it will get better.

Clean, clean, clean. Of all the qualities to possess as a medical professional, cleanliness is one of the most important. Even the best surgeon in the world will be a hazard to their patients if they don't maintain proper hygiene standards. In the world of super viruses and resistant bacteria, it never hurts to wash your hands once more. Your Sim will need the full ten Cleaning points to become Chief of Staff.

Work on those analytical skills. Medicine is a very precise science, requiring the utmost attention to detail and the highest level of logical precision. Your Sim will be an expert at Chess by the time they become Chief of Staff! Your Sim will need all ten Logic points to reach the top of the Medical career.

Do it yourself. Got things blowing up around the house? Don't call the mechanic, fix them yourself! In order to keep up with the latest medical technology and to use the tools at their disposal effectively, your Sim will need to be highly proficient with machines. Your Sim needs nine Mechanical points to become a Chief of Staff.

Get to know your patients. Or people who wander by your Sim's house. Either way, you'll want to make some friends. Office politics can be a dangerous game and it helps to know the right people. Your Sim will need at least nine friends to ascend to Chief of Staff.

Start a business on the side. While it never hurts to have a business to provide some extra income, your Chief of Staff may want to consider it more strongly than most. Though the benefits are excellent, the Medical Career tends to have salaries in the middle range. Ambitious Sims (like Fortune-aspiration Sims) may want the luxury of having some additional capital at their disposal.
