You have just gotten married and need to change your name on your passport. You probably think that this is going to be a complicated, expensive process. The good news is that it is fairly simple to do. All you need is the right form and a little bit of time, and you can easily change your name on your passport. Get started today, though, because you need to take care of this quickly to get your passport back in time for your next trip!

Things You'll Need:

  • Certified copy of marriage certificate
  • Certified copy of name change court decree
  • Certified copy of divorce record
  • Two passport photographs
  • Current passport
  • Mailing envelope
  1. Step 1

    Get a copy of form DS-5504. This form can be used if your name was changed within a year of having your passport issued. This can be printed on-line or you can get it from your local passport office. This office is often found in a major post office.

  2. Step 2

    Get a copy of form DS-82 if your name was changed after a year of having your passport issued. This is a renewal application and can be downloaded on-line or picked up at the passport office.

  3. Step 3

    Order a certified copy of your marriage certificate, divorce documentation, or the name change court decree, as needed. Make sure that you get a copy because you need to keep one of these in your own records.

  4. Step 4

    Fill out the appropriate application completely. If you are using DS-82, you will need to pay $72 to the U.S. Department of State. If you are using DS-5504, there is no fee.

  5. Step 5

    Put the application, certified documents, check (if needed), your old passport, and two passport photos into an envelope. Mail these to the address on the application.

  6. Step 6

    Watch for your changed passport to arrive in the mail.

Tips & Warnings
  • Make sure that you sign and date your application.
  • If your passport has been damaged in any way, you will need to apply in person. This will probably require an appointment.
  • Give yourself plenty of time to get your passport back. This can be a lengthy process, and expedited services cost an extra $60.
  • Do not use photocopies of your marriage license or other legal documents. You must send certified copies.
