Lost or damaged your Social Security card? As long as you still know your Social Security number (SSN), getting a new replacement card is no problem. You can even handle a good deal of the replacement process online, at the Social Security website.

Instructions

1. Fill out the Application For A Social Security Card (Form SS-5), found at the Social Security website. The form will ask basic identifying information (name, address, etc.) along with your Social Security number.

2. Gather proof of identity documents and proof of U.S. citizenship or immigration, if necessary. Acceptable documents for proof of identity include a U.S. driver's license, state-issued identification card or U.S. passport.

3. Take your application form and documents to your local Social Security office for a new card. There is also a mail-in option for replacement cards, though it will mean sending your original identification documents through the mail.
