Step1
Visit the Social Security Administration website and download form SS-5 (Social Security Administration Application for Social Security Card). Or, if you prefer to talk to someone, call the Social Security office and request the form.

Write an explanation. In order to change your social security number you must write a compelling statement as to why you need to change. If you are a victim of domestic abuse, state so in the letter and be sure to enclose copies of the order of protection or any other formal documentation that provides proof of abuse. Also include statements from friends, relatives and co-workers.

Gather the necessary documents. You must bring proof of citizenship or immigrant status. You also must bring proof of age. If, in addition to changing your social security number you have also changed your name, you must bring proof of identity.
Step4
Provide proof guardianship if you are requesting a social security number change for a child. Because of the increase in child kidnappings, it is imperative to bring ironclad proof that you are the child’s legal guardian and have the authority to request the social security number change.
Step5
Bring the necessary documents to the nearest Social Security Administration office. You cannot obtain a new social security number online. This is one of the few remaining services that must occur face to face.
