Things You'll Need:
- Copy of last year's income
- What you paid for rent in the past year if you are a renter
- A copy of your real estate taxes if you are a homeowner
- The amount you have in your checking account, savings, and CDs, etc
- Medicare card

Step 1
Call your local community action agency, township, or city if you are above the age of 61 or if you are disabled to ask about the Illinois Cares Rx program. You must be a resident who lives in Illinois to apply.

Step 2
Decide on a time that would be good to make an appointment. Discuss your options with the agency and how the program may be of beneficial to you.

Bring the following documents with you: a copy of last year's income, how much you have in your checking or savings account, CDs, etc. Also you need your Medicare card, and a copy of your Real Estate taxes if you own your home as well. If you are a renter, bring in how much you paid for rent in the past year and the name and address of your landlord.

Step 4
Apply online with your documentation that you brought in at a local community action agency with a counselor. Ask any questions that you may have. They will be there to assist you.

Wait at least 30 days for a response. You will receive a letter to let you know if you are eligible or if you need additional documentation. If you are eligible, you will get an Illinois Cares RX blue card that will assist you in paying for your medication and also a letter from the Illinois Senator to help you receive a discount on your Illinois license plate.

Step 6
Call your Community Action Agency after February 1st of each year to see if you need to apply. You must apply for the Illinois Cares RX program once a year to receive additional coverage on your prescriptions and to get a discount on your license plate.
