The first step is to measure the door horizontally. You will need to measure across both the top and the bottom of the door. Note any variances because this may require some extra materials or perhaps a special order door. Write down the exact measurements.

Next, measure the door vertically. Measure both sides of the door or doors. Always use the smaller measurements for the doors to insure a proper fit.

Next, you will want to measure the width of the wall. Not all sliding doors are the same width so taking this measurement is key for a proper fit.

Finally, it is a good idea to measure the frame of the door. This is in case you have to special order a door of make adjustments to the frame. In some cases, doors may have been cut too and you will need to find doors to fit the frame rather than just measuring/replacing existing doors.

Last, repeat the entire process to get a second set of measurements. Compare to your first set of measurements. If they match, you have your measurements. If they do not match, measure again.

Tips & Warnings
Sliding doors tend to work extremely well in smaller spaces because no floor space is taken up by the swinging out of the doors.
Standard doors are usually 80 inches tall but this can vary depending on the builder or the age of the home. Most doors can be cut in the store to fit the size needed but occasionally special orders may be needed.
Most doors can be made into sliding doors with the proper sliding door hardware.
ALWAYS measure twice. This follows the old rule for home improvement projects, "Measure twice. Cut once."
Be aware that many doors will not be exactly plumb due to variances in the walls, floors, etc. There are no perfect squares in houses and this is due to a variety of factors including settling.
If the doorway is not square, you will need to level it with either shims or something to level the track. Any evidence of this work can be covered with paint or decorative trim.
