How to Change a Name on the 1040-ES

You must change your name on your 1040 estimated tax payments if you legally changed your name through marriage, divorce or any other situation and previously made estimated tax payments using your former name. Changing your name and notifying the correct officials avoids delays in handling your return and distributing your refund. It also protects any social security benefits in the future.

      Complete your income tax return. This form needs to have all the up-to-date information listed within the document, including all estimated tax payments made for the year that you are filing.

      Attach a statement to the front of your income tax return. The statement should include all of your current information. It should also include the information from Steps 3 and 4.

      Report all estimated tax payments that you made for that year, separating the payments out by quarter. If you are filing jointly, you should also include the tax payments made by your spouse.
