Step 1
To find the nearest Social Security Office to you, please check here: https://secure.ssa.gov/apps6z/FOLO/fo001.jsp. This site is full of great information.

Step 2
To replace a lost Social Security card you must complete an application for a Social Security Card, which you can find online or at your local Social Security Office. You must be able to prove your identity, and citizenship.

Step 3
The application can sometimes be mailed. Talk to your local Office first though before sending off documents. Any documents that are submitted to the Social Security Office must be original, or the documents must be certified original copies or the Social Security Office will not accept the application.
