Step 1
The first step in getting in shape is getting your mind in shape. I know that this may sound a bit odd, but I have always found that it really helps people stay focused if they are motivated. You need to find a reason for getting in shape. Once you have a reason to complete something, the reason may help to keep you on track.

Step 2
The second step to getting in shape is to come up with a plan. You should come up with an exercise routine and diet. Your exercise routine should explain not only when you will workout, but also when you will not work out. A lot of times I see people become discouraged from exercising because of the fact that when they miss a day of exercise they see it as giving up. It is easier to take a day off that was planned. The same goes for dieting. You need to have breaks from when you are on a diet. No one person eats "the right foods" all the time. People are not machines. With that in mind we need to understand that we all need breaks from routines.
When designing your plan, you need to create once that you feel as though will be moderately easy. A lot of people go into panic moods when trying to lose weight or get in shape. This will almost certainly cause burnout. You have to realize that the best way to getting in shape is over a long run and not a short run. You will have more results this way. -
Step 3
Once you have a plan you need to act on it. This is where discipline comes in. You need to keep reminding your self of why you are doing this. Motivation is a big key to obtaining goals. I use motivation in all of my every day life challenges.
