I strongly recommend going through a training to avoid many pitfalls. Just attended the Matchmakers & Dating Coaches Worlwide conference, offered by the Matchmaking Institute. They have a pretty detailed package you can get for about $1,500 (including forms, sample agreement, etc) plus formal trainings in NY, NJ, LA and Singapore... Very nice folks too.
