Men who have sex with men (MSM) are at an increased risk of acquiring infection with the Human Papillomavirus (HPV) much like women who are sexually active. HPV is a hardy virus that can lead to abnormal rectal cells of the anus, and even anal cancer. Although many authorities have recommended that all MSM have a regular anal pap smear--this recommendation has not yet made it into general clinical practice. This article will focus on how to explain to your provider that you need an anal Pap--and what it involves.

Instructions

Difficulty: Moderate

Things You’ll Need:

  • Medical appointment with your primary care NP or MD
Step1
Ideally, MSM should "come out" to their health care providers. This is essential for your health. If you feel uncomfortable with your present health care provider, consider looking for a gay or gay-friendly clinician. One of the best resources is the Gay & Lesbian Medical Association website, glma.org (see link below).

Even if you do not consider yourself to be gay, yet still have sex with other men (a common situation), GLMA is still a good resource for you to seek a savvy provider.
Step2
Do not make a big deal about asking for an anal pap smear. "Normalize" the issue when talking with any provider. Sexual health is critical to everyone's overall general health.
Step3
The anal pap smear is a very simple test and procedure. The clinician will gently insert a special swab in your anus to obtain cells (much like a vaginal pap smear). The cells will be transferred to a slide and prepared for the lab. That is all there is to an anal pap smear.

Tips & Warnings

  • Anal pap smears DO NOT screen for prostate or colon diseases. You will still need to have regular prostate and colon exams.
