  • Recent utility bill
  • Payroll stub
  • Social Security cards
  • Birth certificates
  • Proof of address
  1. Step 1

Contact the county assistance office or the state human services department by phone or by going online. Request an application or print it from the website. Gather your information such as a recent utility bill, proof of income and Social Security cards and birth certificates for each member of the household. Proof of residency is also required, such as a property tax bill, deed or rent receipt.

  2. Step 2

Fill in the general information such as your name, address, phone number and Social Security number at the top of the application. Check off whether you're a homeowner, a renter or a roommate.

  3. Step 3

Identify on the application your main heating source and a secondary heating source if you have one. Choose the fuel supplier where you want the assistance payment to be sent.

  4. Step 4

List everyone who lives in your household, including yourself. Provide information on your monthly income and its source. Remember to attach proof of income, such as a payroll stub.

  5. Step 5

Read the certification statement at the end of the application. Make sure that you've filled in everything on the application that applies to you. Sign and date it, and attach copies of the information listed in Step 1. Mail the application to the correct address.
