How to Replace a Lost Diploma
So you've lost your diploma, or perhaps the dog ate it? A lost diploma can sometimes be difficult to replace, but most high schools and universities will replace it for a small fee.

Contact the high school or college to determine the procedure for replacing a lost diploma. Most schools handle replacements through the registrar's office. Some colleges have forms that you can complete online to request a replacement.

Complete any required paperwork to request a replacement diploma. Include your name, the name used when attending the school, your address, phone, social security number, date of graduation, major (if applicable) and any other information requested.

Provide documentation of a legal name change to change the name on the diploma. Each college has different documentation requirements.

Submit the completed form and payment to the college via regular mail, or online if the school offers that option. Fees can range from as little as $15 for some colleges up to $150 for Ivy League colleges.

Keep an eye on the mail for the replacement diploma. Frame it and hang it, or store it in a safe place.

Tips & Warnings
Some colleges require notarized letters and/or forms to complete the replacement request.
Note that the replacement diploma will have the word "copy" or "re-issued" stamped or printed on it.
Beware of businesses that will replace a diploma for a fee. These are often businesses that will create fake diplomas. Don't waste your money.
Requests can only be completed by the person whose name is on the diploma due to privacy acts.
