Step 1
Think about your book. What is it really trying to say? What is it about? Who are your targeted readers? Once you answer these questions, it is easier to think about your title.
-
Step 2
Should your title have something to do with the book or not? This is a judgment call. Sometimes titles have absolutely nothing to do with the content of the actual book, but due to the uniqueness of the title and the parallel that can be drawn later, the title works well. On the other hand, "On The Road" is clearly about being on the road, and that book did pretty well.

Step 3
Narrow it down to 5 choices. Have a couple titles that apply to the book and a couple that don't. Have several friends and/or colleagues read your manuscript. Ask them what they think is most effective of the 5 titles.
-
Step 4
Factor in their opinion. Think about your marketing plan. Say the titles out-loud as if you were in an interview.
