Health records are required for your child to start school. Locating and arranging these records ahead of time will make starting school easier. Buy some supplies and get organized now.

  • Binder
  • Section dividers
  • Page protectors
  • Immunization records
  • Physical form
  • Dental form
  • Birth certificate
  1. Step 1

    Purchase a 3-ring binder, section dividers and some page protectors. Make sure the binder is sturdy enough to last a few years.

  2. Step 2

    Learn health requirements for students in your state. Every state has its own requirements. The fastest way to learn what documentation is needed is to contact your local school system.

  3. Step 3

    Make a list of the health requirements that are appropriate for your child. Usually this will include a recent physical examination, a dental examination, and immunizations that are up to date for your state requirements. A birth certificate is almost always required for school entry. Even though this is not technically a health requirement, it makes sense to store a copy of this document with the health records. Add this item to your list also.

  4. Step 4

    Locate these documents. Make sure the required records are up-to-date and are using forms approved by your school district. If you need specific physical forms and dental forms, request them from the school.

  5. Step 5

    If your child is behind on any of the required health services, schedule the appointments as soon as possible. Bring the required forms with you to the scheduled appointments, to ensure they are signed and returned to you quickly.

  6. Step 6

    Place the forms into page protectors and put them into the appropriate category. As your child’s records are updated, simply add the most current record to the book. You will always be ready for any situation that requires health information.

Tips & Warnings
  • Keep the book with your child’s records in a safe location. A fireproof filing cabinet is a good investment to make if your family does not already have one.
  • Keep original birth certificates and a copy of immunization records in a safe deposit box for maximum protection.
