Things You’ll Need:
- Birth Certificate for EACH person for whom you are applying for
- Photo ID for anyone 17 and older you are applying for
- Last 4 weeks of income information (if there is no income you will need a written statement from anyone who is providing you with assistance to live on)
- Pregnancy verification with estimated date of delivery from doctor or health department if applying for pregnancy medicaid
Step1
You should apply in person-the documents listed must be shown as originals to a worker. Apply at the county office in which you live. If you are unsure of the location you can call 1.800.549.0820 for information and locations
Step2
Call your local office to verify when you should arrive for an application and interview. Most offices are open Mon-Fri 8:30-5:00 but accept only a certain amount of applications daily beginning at 8:30 a.m.
Step3
For those who are:
Blind
65 or older
Disabled -as determined by the Social Security Administration
the programs and documentation differs somewhat. Call your local office to set up an interview.
