- Form SS-4 and instructions
- Scanner or photocopier

Step 1
Read the instruction booklet that comes with form SS-4. The instructions provide line-by-line directions and examples as you fill out the form.

Use a black or blue ballpoint pen to fill out your application. Dark colors are easier to read than red or other novelty inks.

Complete the first section of the form that asks for the legal name off the business and where it is located. If you own a home-based business, this would be your home address.

Check off the type of entity that is applying for the EIN. Entities include sole proprietors, partnerships, corporations, churches, estates, trusts and governments. If you do not know what type of entity you are dealing with, refer to the instruction booklet.

Step 5
Have business records available when you begin to fill out form SS-4. You will need to know the exact date the business started, the dates of the accounting year and the first date on which the business started to pay wages to its employees. If your business does not employ anyone other than yourself, you can write "N/A" on that line.

Step 6
Explain the type of banking for which you need an EIN. For example, if you deposit soccer club dues to a special account, write it down here.

Choose the division of labor that best describes your business. If none of the choices given fit the services you provide, write it in the space allotted.

Sign and date your copy of form SS-4. Scan it and save the file on your computer or make a photocopy. Save it in a safe place until your EIN arrives.

Mail your completed form SS-4 to the regional address that is given in the instruction booklet.
