How to Create Work Accident Injury Report Forms

If you are a small business owner, part of the job is to submit various reports to your local government and keep current with licensing. If you have employees working for you and they hurt themselves on the job, you will have to file injury report forms that describe how, when and where the injury occurred. Generic templates for work accident injury report forms are available, but it is also easy to make your own personalized forms.

  • Business forms templates

Step1
Create a personalized graphic for your business that is easily recognizable. This could be a logo, a motto or just your business name written out in a distinctive font. Use your computer to generate the logo or draw it by hand.
Step2
Add your logo to a blank word processing document at the top or corner of the page. This can be a template for business stationery, as well as the different report forms you may be required to keep.
Step3
Find a template of a work accident or injury report form that is suitable for your business. Findlaw.com has a collection of small business forms online or you can purchase a book of business forms from Amazon, such as Lynne Anne Frasier's "The Complete Book of Business & Legal Forms" (see Resources below). These forms will help you know what questions to ask and how to phrase them.
Step4
Use the template you have chosen as a guide as you type up your own work injury and accident report forms. Certain information is required, such as the employee's name, social security number and birth date. Leave plenty of space for the injured party to explain exactly what happened during the accident.
Step5
Create an "authorized personnel only" section at the bottom of your form. This is for the management (you, if you are the business owner) to report what actions will be or have been taken regarding the work-related accident. Enclose this section in a box or shade it another light color so that the employee will know not to fill it in himself.
Step6
Print out a copy of your report forms and proofread. Determine if it looks official and make changes and corrections as necessary.

Tips & Warnings

  • Less is generally more when you want to create official-looking business forms. Try to keep the wording concise to avoid confusion as people fill in their information. A small business seal or logo is appropriate but other graphics should be avoided.
