IRS form 8109 is a machine-readable deposit ticket used to pay quarterly taxes. After receiving their Employer Identification Numbers (EIN), businesses are sent coupon booklets that are preprinted with their specific information. Individuals must fill out form 8109 very carefully to ensure that the correct numbers will be read and the right amount of taxes is being paid.

Things You’ll Need:

  • Form 8109 and instructions
  • Ballpoint pen
  • Number 2 pencil
  • Scanner or photocopier

Step1
Use one of the preprinted forms you received for your business. You should have received a packet shortly after you got your Employer Identification Number (EIN). If you do not have any more preprinted forms, you will need to use a form 8109-B until you receive your new supply.
Step2
Fill out the name and address information section of the deposit ticket in pen. A black or blue ballpoint pen is easily read and less likely to bleed than a felt-tip pen. If you choose, you may complete this section only on a typewriter or printer.
Step3
Use a soft-leaded, number 2 pencil for the remainder of the form so it can be read correctly by the machines.
Step4
Write your numbers as shown in the instruction portion of the form to make sure they are read accurately. For example, the number 1 should just be a straight, vertical line without a horizontal line at the bottom and tail at the top.
Step5
Fill out your EIN, the month that your tax year ends and the amount of your tax owed. Omit dollar signs when entering the tax amount.
Step6
Shade in the ovals that signify the type of tax you are paying and the tax period covered. Make sure you have filled in the ovals completely. The instructions can help you determine what type of tax you are paying if you are not sure.
Step7
Keep a copy of your payment for your records.
Step8
Send your form 8109 and tax check to the address listed on your coupon booklet.

Tips & Warnings

  • If you have no more of your business forms 8109, contact the IRS directly to get a form 8019-B. Because the form is machine-readable, it is made with special paper and ink and cannot be downloaded from the Internet. The IRS website has information about how to obtain form 8109-B (see Resources below).
