How to Fill out Unemployment Forms

There are more than 10 different forms you may need to fill out if you are applying for unemployment benefits. Which ones you need depend on many factors, including whether or not this is your first application, where and how much you worked over the past 18 months, your earnings and whether or not you are a US citizen.

Step1
Follow the instructions provided with each form carefully. Some forms may need to be duplicated or have special requirements. Failing to fill out the forms the correct way may lead to a delay.
Step2
Fill out the UIA 1554-M Application for Unemployment Benefits to get started. This form asks for your personal data, including your social security number, address and phone number, the number of dependents you are claiming and whether or not you want taxes withheld. If you are unsure of an item, read the instructions that come along with the forms. Changing your information later on can lead to delays and to the suspension of your benefits.
Step3
Make sure your full name and social security number are included on each and every page of the application. This is required to verify your identity and a detail many people forget when they fill out the forms. If you are not an American citizen or do not have a social security number, make sure you note this on the first page and then complete the required extra forms.
Step4
Add the UIA 1509 EFC, Alien Consent of Disclosure if you are not an American citizen. This form allows the unemployment office to check for your immigration status and verify that you qualify for benefits. As you fill out this form, make sure you include your alien registration number and what type of ID you are using.
Step5
Provide details on the reason you lost your job. Forms will give you a variety of choices, some of which actually disqualify you for unemployment benefits (such as quitting or retiring). Every time you choose the option "other," you will need to explain in your own words why. Always be brief and to the point and avoid giving excuses or blaming anybody.

Tips & Warnings

  • Write all information in block letters that are clearly legible. Many forms are returned and claims denied because workers cannot read the information entered.
  • Make sure you complete both sides of the forms without skipping over any particular point.
  • Most forms require that you use only black ink.
