From Quick Guide: Change Your Name
Difficulty: Moderate
Instructions

Things You'll Need:

  • Form DS-82 from the Bureau of Consular Affairs
  • Pen
  • New passport photos
  • $67
  • Certified copy of a legal document showing your name change
  1. Step 1

Determine which form you will need. If you haven’t applied for a passport within the past year and can provide a certified copy of your marriage certificate you will likely qualify to use form DS-82. In order to use this form and apply for a passport renewal by mail you must have a passport that is not damaged. Also you must have received the passport within in the past 15 years and were at least 16-years-old when it was issued.

  2. Step 2

Obtain form DS-82. You can pick this up at your local post office, or print it off from the Bureau of Consular Affairs website.

  3. Step 3

Fill out the form. You must include new passport photos. You can get these at your local drug store such as Walgreens, CVS or Rite Aid.

  4. Step 4

Attach your completed form to your most recent passport and include a check for the required amount, which in most cases is $67. Place all items in a padded envelope.

  5. Step 5

Mail your completed form to: National Passport Processing, P.O. Box 13349
    Philadelphia, PA 19101-3349.

Tips & Warnings
  • If you plan to travel soon, apply for a rushed application. This will require an additional $60 application fee and you must clearly write “expedited” on the envelope.
  • Allow several weeks for your new passport to arrive.
  • Payment must be in U.S. dollars.
