In Illinois, to receive a disabled license plate or placard, you must fit into one of five conditions: * Cannot walk without the use or aid of cane, prosthetic, wheelchair, etc. * Restricted by lung disease * Uses portable oxygen * A cardiac condition * An arthritic, neurological or orthopedic condition as determined by a licensed physician

Things You’ll Need:

  • A Persons with Disabilities Certification for Plates or Parking Placard Form (VSD-62.18) signed by your doctor.

Step1
Make appointment with your doctor about your condition to determine if you are eligible for the disability plate or placard, and if they are willing to sign the form.
Step2
Obtain a Person with Disabilities Certification for Plates and Parking Placard from the Secretary of State's Office, either in person or on-line.
Step3
Get your doctor's signature.
Step4
Submit form to the Secretary of State's Office.
Step5
Place license plate on your vehicle or for placards, display them when you are using handicap parking.

Tips & Warnings

  • If you feel that you may need a plate or placard, consult your doctor. You may need to justify why you need it, if your doctor is reluctant to sign the form.
  • Remember, you and/or doctor may be fined for falsifying any information.
