How to Fill Out IRS Form 1099-PATR

IRS Form 1099-PATR, entitled "Taxable Distributions Received From Cooperatives," is sent to taxpayers who received money from a cooperative. This article is written from the point of view of the entity filling out the 1099-PATR, not the person receiving it.

Instructions
  1. Step 1

    Call (800) TAX-FORM or visit the IRS online form ordering website (see resources below) to obtain acceptable copies of Form 1099-PATR to send to recipients. Although Form 1099-PATR is available for viewing online, the e-version of the form is unacceptable to send to taxpayers or the Internal Revenue Service.

  2. Step 2

    Check the "Void" or "Corrected" box at the top of the 1099 form if applicable. Enter the Payer's name, street address, city, state, ZIP code and telephone number in the first box. The Payer is the entity who distributed the taxable cooperative money.

  3. Step 3

    Enter the Payer's Federal identification number and the Recipient's identification number (usually a Social Security number or an EIN) in the two boxes beneath the first box. Below this, list the Recipient's name, street address (including apartment number), city, state and ZIP code. If applicable, enter the account number that is associated with this transaction. Check the box if the Recipient has a second tax ID number.

  4. Step 4

    List patronage dividends paid to the Recipient in cash, qualified written notices of allocation (at stated dollar value) or other property in Box 1. In Box 2, show the non-patronage distributions paid to the Recipient in cash, qualified written notices of allocation or other property. Indicate the per-unit retain allocations paid to the Recipient in cash, qualified per-unit retain certificates or other property in Box 3. In Box 4 list the backup holdings for the Recipient; if he or she did not provide you with a tax ID number, you must withhold 28% of any distributions.

  5. Step 5

    Show the amounts the Recipient got when he or she redeemed nonqualified written notices of allocation and nonqualified per-unit retain allocations in Box 5. In Box 6, demonstrate the deduction amount the Recipient may take based on his or her portion of patronage dividends or per-unit retain allocations attributable to qualified production activities income. List any investment credit or work opportunity credit for which the Recipient is eligible in Boxes 7 and 8. Show the AMT (alternative minimum tax) percentage passed through to the Recipient in Box 9. In Box 10 list any other credits or deductions to which the Recipient is entitled.

  6. Step 6

    File Form 1099-PATR with the IRS; also send a copy to the recipient. Check the IRS official website (see resources below) for specific instructions as to the filing deadline for Form 1099-PATR; it changes annually.
