1
Open a new document using any word processing software. Center the cursor at the top and type, "New Employee Network Setup Form."
-
2
Insert a table with 3 columns and approximately 13 rows. Add "Information," "(Please Fill Out Below)" and "Description" as the column headings.
-
-
3
Create titles for the rows depending upon the particular business. Standard ones include "Start Date," "Name," "Title," "Workstation Requirements," "Applications Needed," "Other Applications," "Office Location," "Phone Setup," "Sales Agent," "Agent Groups," "Portal Access" and "Other Requirements." Leave the second column, "(Please Fill Out Below)" blank until the actual setup occurs.
-
4
Add explanatory sentences in the third column, "Description", for each row mentioned above. "Start Date," "Name" and "Title" are self-explanatory. In the cell for "Workstation Requirements" add "Standard, high performance or laptop - Lead time required for new hardware." The descriptive section for "Applications Needed" should explain which applications have been pre-installed.
-
5
Put in explanatory text for "Office Location" to indicate where to setup the workstation and phone. Describe "Phone Setup" as, "Agents can have calls routed to them via the call queues." For "Sales Agent" explain whether or not the employee will be a sales agent in the portal. Include a description of "Agent Groups" that explains whether they be sales, support or service and to what groups they should belong.
-
6
Finish the form by formatting its appearance. Choose a neutral color for shading the row. Change the font within the row to boldface type.
