Spa licenses are issued by the state, and in many states a spa is just another name for a salon. Spas and the different professionals working within the spa are required by law to be licensed in order to protect the health and safety of their customers. The requirements for the employees vary depending on what their specialty is. A complaint is all that is required to report a spa for an expired license.

Step1
Ensure that the spa's license is indeed expired. It should be displayed in a prominent area. It is also an offense if it is not displayed.
Step2
Gather all of the information about the spa, such as exact name, address, city, state, zip code and telephone number. Note the date(s) that you know the spa's license was expired.
Step3
Locate your state's board responsible for licensing spas. A good place to start would be your Secretary of State's office, which should be accessible online. That office should be able to get you in touch with the proper licensor, which will often be a cosmetology or massage licensing board for the state.
Step4
Fill out a complaint. Sometimes, this can be done online. You need to provide the information gathered in Step 2. In addition, some complaint forms allow you to write in your own words what date you were there and any other information you feel is important for reporting a spa for an expired license.
Step5
Wait for a response from the licensing board that your complaint has been received. The board will take it from there. Usually the fines are not extremely high for the first offense, but at least the spa will now be under the looking glass by the state.
