Enter the name of the workshop or seminar you plan to attend. Include the date(s), times(s), city and state of the class.
Step4
Provide complete attendee information, including name, agency or company name, mailing address, city, state, ZIP code, telephone and fax numbers and email address.

Indicate if you would like a certificate for CPE (continuing professional education) credit purposes. Include any additional comments you may have about your upcoming seminar or workshop.
Step6
Check your email regularly. This is the method that the IRS uses for communication about matters such as registration confirmation, directions to meeting locations, payment information and any special accommodations you may require.
