How to Fill Out a Group Health Insurance Application
Group health insurance is one of the benefits many employers offer to attract quality employees. As healthcare premiums continue to rise, employers are taking steps to reduce the cost of health insurance to the company and the employee. Many options are available through group coverage that will affect the premium the employee pays and the amount required from the employer.

Provide Complete Information on the Application

Consider the deductible options available. The premium for a higher deductible such as $1,500 will be less than for a deductible of $250. For policyholders in excellent health, the higher deductible is often a good option.

Fill out the date of birth and Social Security number accurately. This information will be used to identify your coverage and claim status throughout the term of the policy.

Determine who will be carried on the policy. Birthday, sex, Social Security number and full name will be required for every dependent.

Identify your physician and clinic of choice if requested on the application. Some group policies limit the choices for healthcare providers, while others allow freedom to choose or change doctors at any time.

Provide information about any other group coverage you may have (such as Medicare) if it is requested. Information you will need may include the group number, policy number and name of the insurance company and plan.

Identify the add-ons you desire with the basic coverage. Many group policies offer dental and vision as options. The employer may or may not choose to pay a portion of the premium cost for added benefits.

Indicate if you've been covered by another group plan within the past 63 days. HIPAA has specific requirements about preexisting conditions and enrollment in a new group plan. Learn about these requirements at the U.S. Department of Labor website (see Resources below).

Sign the application when it is completed. The signature on the application indicates that all information is true and complete. The lack of a signature will delay processing the application.

Select a lower cap for payout to reduce the health insurance premium. If the option is available, choosing a cap such as $500,000 instead of millions of dollars will lower the amount of cost for the employee and employer.
