Step1
Determine if you have become a victim of someone violating a law of the ADA. Review the terms of the Americans with Disabilities Act by visiting the ADA website, or contacting legal counsel to help you navigate the terms of the ADA.

Write down the facts of the violation while it is still fresh in your mind. Who did you speak with, what time of day was it, what were the situations surrounding the time of the violation? Write down everything you remember, so you can better recall what happened after the fact.
Step3
Contact the ADA Information Line. Whether you or a close friend or relative calls, ask plenty of questions, and describe your dilemma. They can often help you to file a complaint, and will give you specific instructions as to what title number of the ADA the violation falls under.
Step4
File the violation report. Visit the ADA website to print out the violation form, or contact the U.S. Department of Justice to have them send you one in the mail. If you need help with the form, call the ADA information line for assistance or ask a lawyer for help.
Step5
Seek legal advice. It is always best to protect yourself or someone you love against further harm by having someone help you navigate the legal issues after you report the violation. You may even find yourself in litigation over the offense, so bringing a lawyer in early on will keep you prepared for any eventuality.
