Step1
Determine if you can create the survey yourself or if will need to hire someone to create it for you. You can create the form yourself or hire someone to create it using HTML. After it's created, post the web survey on your website. Design it to email you the results and store the information in a database.

Research survey software if you have determined that you would rather use a software platform to deliver your web survey. You have the choice of buying software that you load onto your desktop or server or software with the platform residing on the vendor's website.

Create a rough outline of the questions you would like to see on your web survey. Have several people proofread the questions to make sure that you don't have any spelling or grammar mistakes.

Decide the process you will use to let the visitors answer the survey. Make a decision determining if you are looking for visitors to answer questions in a yes or no fashion, by choosing one answer among multiples or to type in answers in sentence format.

Plan on how you will encourage the visitors to take the survey. Decide if you will have the survey as a pop-up, send the link through email to repeat customers or invite visitors to take the survey in some other way.

Put the survey on your website.

Test your survey to see the experience visitors have in taking your survey. Ask some colleagues, friends or family to take your survey and give you some feedback. You want to make sure that it doesn't take too long, it's easy to read and understand and the visitor has a pleasant experience when taking the survey.
